@RR There are several stakeholders who caused this.

#1 being the structural engineering company who did the design. Probably zero investigation into the structure (to save money).

#2 being the developer putting pressure on the master engineering firm (JMV Associates) and the Prime contractor to meet a deadline.

#3 Prime contractor's "means and methods" to do the work. A lot of times they cut corners and use this as an excuse, bypassing engineering standards.

#4 "Independent inspection or 3rd party inspectors". Usually pretty worthless. They show up to the site for a short while and bitch about tiny ticky tacky shit while ignoring those YUGE CRACKS IN THE COLUMNS. Then go do a 3-hour lunch.

#5 Master engineering firm (Engineer of Record (EOR). Takes all the credit (and money) because they are a minority owned business that doesn't do any of the work (farms it out). Blames everyone else if the project fails.

These f@ck ups usually seemed to be caused by the under funded and poorly manned crews onsite assuming that everyone before them in the process engineered in infinite redundancy to make up for their incompetence (alas they're all incompetent now).
